Risk Awareness Form

  1. Please read and acknowledge our Risk Awareness policy

    "I Understand that some of the activities in which I/we may wish to or be participating in, can be hazardous and dangerous

    I Understand that the activities in which I am willing to participate in will be described to me in sufficient detail to enable me to understand the risks that will be involved
    I Understand that I am solely responsible for my decision to allow my child/ward to participate having considered the risks
    “volenti non fit injuria” meaning; - No injury is done to one who voluntary accepts a risk
    I understand that I am under no obligation to continue activities and that I/we can withdraw at any point and time of my/our choosing
    I declare that I /we are not under the influence of medication that may affect my/our abilities to participate or knowingly place others at risk from my/our acts and omissions whilst taking medication
    I declare that I am not under the influence of alcohol or illegal substances or legal highs and or hallucinogenic substances
    Motorised activities
    I declare that my eyesight (with or without corrective lenses) is of a standard required to drive a car in the UK
    I declare that I and or the wards in my charge or under my supervision is sixteen years of age or over on this day, I confirm that all participants are in good health and have no outstanding medical condition which would be dangerous for them to participate in these events. I am not aware that any participant suffers from epilepsy, fits, and fainting spells or has any pre-existing back, neck or spinal complaint.
    Note: certain restrictions may apply on the grounds of weight or physical ability to participate without significantly increasing the risk of harm to yourself or to others, and are at the sole discretion of the instructor’s assessment of the participant’s capabilities. It is an offence for someone who is prohibited by Section 21 of the Firearms Act 1968 to have a firearm or ammunition in his/her possession at any time. Any injuries must be declared before departing. Images may be taken of groups that could include children, complete our image consent form if this is of concern.
